multiple sclerosis (MS) (encephalomyelitis disseminata)

Definition of

Disease of the myelin sheaths, they die off (demyelination foci) and are replaced by glial scars (sclerosis). 2nd form: chronically progressive and relapsing forms. There are often precursors: occasional paresthesia, visual disturbances (blurred vision, nystagmus (flickering), balance disorders, unsteadiness of movement)

Cause

  1. Unknown, presumably autoaggressive or effect of (persistent ?) measles viruses (a high measles antibody titer was frequently observed); increased permeability of the blood-brain barrier through which T lymphocytes migrate and destroy the myelin sheaths. Mostly affects women, usually 20-40 years of age.

Diagnosis

  1. CT or MRI is necessary for a reliable diagnosis
  2. the laboratory is o.B.
  3. earliest onset: positive L’Hermite sign (arms stretched out to the side, head towards the sternum -> hands begin to tremble)
  4. afterwards: Abdominal wall reflex fails
  5. positive Babinski sign

Symptoms

  1. Highlight: Intention tremor (becomes shaky shortly before reaching an object)
  2. unilateral neuritis of the optic nerve
  3. Irregular, non-symmetrical sensory disturbances
  4. Increasing leg weakness -> spasticity -> ataxia -> total paralysis
  5. Vertigo (dizziness)
  6. Nystagmus
  7. chanting language (choppy, empty)
  8. Change of character
  9. euphoric indifference
