multiple sclerosis (MS) (encephalomyelitis disseminata)
Definition of
Disease of the myelin sheaths, they die off (demyelination foci) and are replaced by glial scars (sclerosis). 2nd form: chronically progressive and relapsing forms. There are often precursors: occasional paresthesia, visual disturbances (blurred vision, nystagmus (flickering), balance disorders, unsteadiness of movement)

Cause
- Unknown, presumably autoaggressive or effect of (persistent ?) measles viruses (a high measles antibody titer was frequently observed); increased permeability of the blood-brain barrier through which T lymphocytes migrate and destroy the myelin sheaths. Mostly affects women, usually 20-40 years of age.
Diagnosis
- CT or MRI is necessary for a reliable diagnosis
- the laboratory is o.B.
- earliest onset: positive L’Hermite sign (arms stretched out to the side, head towards the sternum -> hands begin to tremble)
- afterwards: Abdominal wall reflex fails
- positive Babinski sign
Symptoms
- Highlight: Intention tremor (becomes shaky shortly before reaching an object)
- unilateral neuritis of the optic nerve
- Irregular, non-symmetrical sensory disturbances
- Increasing leg weakness -> spasticity -> ataxia -> total paralysis
- Vertigo (dizziness)
- Nystagmus
- chanting language (choppy, empty)
- Change of character
- euphoric indifference
