Baastrup’s disease, Morbus Baastrup (Baastrup-syndrome, kissing spine desease)

Definition

Chronic lumbar pain syndrome caused by contact with the spinous processes and irritation of the corresponding ligaments of the lumbar spine

Predisposing

  1. Hypertrophically developed spinous processes, possibly as a result of hard physical work
  2. heavy physical work, especially jobs involving shoveling, such as road construction
  3. Degenerative changes in the spine with loss of distance between the vertebral bodies, especially ventrally
  4. aseptic bone necrosis of the vertebral bodies

Diagnosis

  1. X-ray may show osteophytes, calcifications, sclerosis of the paravertebral musculature
  2. Lumbar spine hyperlordosis

Symptoms

  1. Spinous processes sensitive to pressure and knocks
  2. Stretching/extension of the spine worsens
  3. Passage analgesia after infiltration of local anesthetic and cortisone
  4. Shaking pain

Therapy

  1. Elimination of muscular imbalances, strengthening of the back muscles
  2. Analgesics
  3. Heat therapy
  4. Electrotherapy
  5. Injections with local anesthetic and cortisone
  6. Only in cases of therapy-resistant pain surgery with creation of a suitable wedge shape of the vertebrae (reduction osteotomy) or removal of the spinous processes
  7. Trunk orthosis
