Baastrup’s disease, Morbus Baastrup (Baastrup-syndrome, kissing spine desease)
Definition
Chronic lumbar pain syndrome caused by contact with the spinous processes and irritation of the corresponding ligaments of the lumbar spine

Predisposing
- Hypertrophically developed spinous processes, possibly as a result of hard physical work
- heavy physical work, especially jobs involving shoveling, such as road construction
- Degenerative changes in the spine with loss of distance between the vertebral bodies, especially ventrally
- aseptic bone necrosis of the vertebral bodies
Diagnosis
- X-ray may show osteophytes, calcifications, sclerosis of the paravertebral musculature
- Lumbar spine hyperlordosis
Symptoms
- Spinous processes sensitive to pressure and knocks
- Stretching/extension of the spine worsens
- Passage analgesia after infiltration of local anesthetic and cortisone
- Shaking pain
Therapy
- Elimination of muscular imbalances, strengthening of the back muscles
- Analgesics
- Heat therapy
- Electrotherapy
- Injections with local anesthetic and cortisone
- Only in cases of therapy-resistant pain surgery with creation of a suitable wedge shape of the vertebrae (reduction osteotomy) or removal of the spinous processes
- Trunk orthosis
