contusion
Definition of
blunt force trauma, i.e. a fall, blow, impact or collision. Does not have to be visible from the outside, usually hardly any injury to the skin. Small vessels rupture, resulting in a painful hematoma. Collagen fibers of the connective tissue also tear. One of the most common sports injuries. Distinguish
- Muscle contusion: formation of a hematoma between the individual muscle fibers
- Bonebruise: Formation of a hematoma between bone and periosteum
- Joint contusion (see there)
- Soft tissue contusion: formation of a hematoma in the skin and possible damage to internal organs, e.g. rupture of the spleen
ICD S00 – S90, T14
Cause
- direct trauma
Symptoms
- Hematoma
- Tissue edema
- Moderate to severe pressure pain
- Pressure soreness
- Painful movements
- In case of joint contusion: painful restriction of movement
- for rib contusions: Pain when breathing (DD rib fracture, pneumothorax)
Complications
- Compartment syndrome
- In the case of muscle contusion: soft tissue ossification (ossification: „heterotopic ossification“)
Therapy
- PECH:
- no massages !!!
- Analgesics: Paracetamol, ibuprofen, diclofenac
- Usually heals spontaneously within approx. 6 weeks, most of the pain is gone after just a few days
- Severe contusions may require surgical intervention
NHK
- Arnica as globules and locally as tincture
DD
- Fractures
- Joint injuries
