Definition
Insertional tendinopathy of the quadriceps as overuse syndrome
Cause
Predisposing
- Runners, jumpers, sports with frequent changes of direction or stop and go, generally more frequent use of high muscle performance in eccentric contraction
- Skiers at the start of the season
- Cycling with the saddle set too low
Diagnosis
- Sono
- MRI: thickening of the tendon and peritendinous edema, signs of degeneration
Symptoms
- Load-dependent pain in the insertion area of the quadriceps tendon
- Pressure soreness
- Possibly local swelling
Complications
- Rupture of the quadriceps tendon
Therapy
- Cryotherapy
- Electrotherapy
- PT: Massage with cross-frictions
- Ultrasound therapy
- Training break or modification of the load/temporary change of sport
- Taping
- Infiltration of platelet-rich plasma
- Working through muscular imbalances, strengthening the antagonists
- Gentle muscle building, especially with eccentric contraction
- In the event of a tear: surgery, then 6 weeks immobilization in an extended position, followed by exercise therapy
