whiplash injury (cervical spine distortion, whiplash injury, WAD whiplash associated disorder)

Definition of

Acceleration trauma to the soft tissues of the cervical spine possibly with ligament damage, most traumas heal within weeks, sometimes it becomes chronic (at least 6 months) as WAD (whiplash associated disorder), neck pain and restricted flexibility in the acute phase worsen the prognosis; differentiate in degrees of severity according to the Quebec classification (levels 0 and 4 can also be omitted):

  1. 0: no complaints, no symptoms
  2. 1: Neck pain, stiffness of the neck
  3. 2: Discomfort and muscle tension, restriction of movement, muscle hard tension
  4. 3: Complaints and neurological findings
  5. 4: Fractures / dislocations

Cause

  1. Traffic accidents (rear-end collision, side collision), ,
  2. Diving, diving into shallow waters
  3. Martial arts

Diagnosis

  1. Medical history
  2. Clinic
  3. MRI

Symptoms

  1. In severe cases, unconsciousness, nausea, vomiting
  2. Dizziness
  3. Drowsiness and attention deficit disorder
  4. occipital headache, burning or stabbing, radiating from the neck towards the head
  5. Hearing disorders/visual disorders, tinnitus
  6. Disorientation
  7. Rapid exhaustion, feeling of weakness
  8. Pain/discomfort in the face and arms
  9. Gait instability
  10. Muscle dysfunction, swallowing disorders
  11. Spasms
  12. Sleep disorders

Complications

  1. Paraplegic symptoms
  2. cauda equina symptoms
  3. Chronification to WAD

Therapy

  1. KG
  2. NO neck brace !
  3. Psychoeducation
  4. Analgesics
  5. NSAIDS
  6. Lidocaine
  7. Antidepressants
  8. Cortisone boost
  9. For paraplegic symptoms OP
