ankle joint arthrosis

Definition

Osteoarthritis of the ankle joint, due to injury rather than wear and tear, usually in the 3rd or 4th decade of life; the cartilage here is only 1 mm thick with a bearing surface of 3.5 cm².

Cause

– traumatic

  1. previous ankle fracture, calcaneus fracture or talus fracture
  2. previous supination trauma (87% of all athletes suffer ligament trauma at some point)

– Dispositional diseases

  1. Juvenile osteochondrosis dissecans with deficiency supply
  2. Secondary to rheumatic diseases
  3. Hemophilia
  4. Infections of the joint

– Musculoskeletal system

  1. Unstable ligamentous apparatus, e.g. due to torn ligaments
  2. Cartilage damage
  3. Malposition of the calcaneus

Diagnosis

  1. X-ray

Symptoms

  1. Start-up pain
  2. Malposition
  3. Pain, stabbing or pulling, also radiating distally or proximally
  4. sudden pain when walking on uneven ground due to pronation and supination
  5. Posture, limping, loss of muscular strength and stability
  6. Reduced resilience and fitness for sport
  7. Shortening the walking distance
  8. later pain at rest

Complications

  1. Painful deformation of the joint and stiffening

Therapy

  1. Conservative: orthopaedic metatarsal roll, custom-made shoes
  2. Infiltration therapy: cortisone, hyaluronic acid
  3. Arthroscopy
  4. Ligament refixation, ligamentoplasty
  5. TEP
  6. as a last option: arthrodesis
  7. Procedures known from the knee joint are slowly being transferred to the ankle joint: transplantation of cartilage cells or cartilage-bone cylinders
