ankle joint arthrosis
Definition
Osteoarthritis of the ankle joint, due to injury rather than wear and tear, usually in the 3rd or 4th decade of life; the cartilage here is only 1 mm thick with a bearing surface of 3.5 cm².

Cause
– traumatic
- previous ankle fracture, calcaneus fracture or talus fracture
- previous supination trauma (87% of all athletes suffer ligament trauma at some point)
– Dispositional diseases
- Juvenile osteochondrosis dissecans with deficiency supply
- Secondary to rheumatic diseases
- Hemophilia
- Infections of the joint
– Musculoskeletal system
- Unstable ligamentous apparatus, e.g. due to torn ligaments
- Cartilage damage
- Malposition of the calcaneus
Diagnosis
- X-ray
Symptoms
- Start-up pain
- Malposition
- Pain, stabbing or pulling, also radiating distally or proximally
- sudden pain when walking on uneven ground due to pronation and supination
- Posture, limping, loss of muscular strength and stability
- Reduced resilience and fitness for sport
- Shortening the walking distance
- later pain at rest
Complications
- Painful deformation of the joint and stiffening
Therapy
- Conservative: orthopaedic metatarsal roll, custom-made shoes
- Infiltration therapy: cortisone, hyaluronic acid
- Arthroscopy
- Ligament refixation, ligamentoplasty
- TEP
- as a last option: arthrodesis
- Procedures known from the knee joint are slowly being transferred to the ankle joint: transplantation of cartilage cells or cartilage-bone cylinders
