Tendopathy / Tendinopathy

Definition of

Prolonged overloading or incorrect loading causes degenerative tears, which are replaced by hypervascular, hypoelastic, often painful scar tissue. Common examples are

  1. PFPS (Patello-Femoral Pain Syndrome, Jumper’s Knee)
  2. Pes-anserinus syndrome: pes-anserinus tendinitis / bursitis / gracilis syndrome, mainly in older women with knock-knees, only occasionally in men
  3. ITBS (Ilio-Tibial-Band-Syndrome, Runner’s knee)

Cause

  1. Prolonged overloading and incorrect loading

Predisposing

  1. Axial misalignments, deformities
  2. Overuse

Therapy

  1. Load reduction
  2. Orthoses
  3. Counterforce bracing for epicondylitis humeri radialis
  4. Initially: short-term use of NSAIDs
  5. for enthesiopathies of the supraspinatus tendon and golfer’s elbow: topically applied nitroglycerin reduces pain at rest and pain on exertion
  6. Caution with corticosteroid injections, they can induce tendon necrosis with ruptures and aseptic tissue damage
  7. Injection of platelet-rich plasma
  8. Hyaluronic acid injection
  9. Extracorporeal shock wave therapy (ESWT), especially PHS and tennis elbow
  10. Sclerotherapy by injection of alcohols such as polidocanol for neovascularization in the paratendinous tissue
