Definition of
Prolonged overloading or incorrect loading causes degenerative tears, which are replaced by hypervascular, hypoelastic, often painful scar tissue. Common examples are
- PFPS (Patello-Femoral Pain Syndrome, Jumper’s Knee)
- Pes-anserinus syndrome: pes-anserinus tendinitis / bursitis / gracilis syndrome, mainly in older women with knock-knees, only occasionally in men
- ITBS (Ilio-Tibial-Band-Syndrome, Runner’s knee)
Cause
- Prolonged overloading and incorrect loading
Predisposing
- Axial misalignments, deformities
- Overuse
Therapy
- Load reduction
- Orthoses
- Counterforce bracing for epicondylitis humeri radialis
- Initially: short-term use of NSAIDs
- for enthesiopathies of the supraspinatus tendon and golfer’s elbow: topically applied nitroglycerin reduces pain at rest and pain on exertion
- Caution with corticosteroid injections, they can induce tendon necrosis with ruptures and aseptic tissue damage
- Injection of platelet-rich plasma
- Hyaluronic acid injection
- Extracorporeal shock wave therapy (ESWT), especially PHS and tennis elbow
- Sclerotherapy by injection of alcohols such as polidocanol for neovascularization in the paratendinous tissue
