traumatic patellar dislocation
Definition of
In addition to patellardislocation, which occurs as a result of an excessively lax inner retinaculum and muscular imbalances, there are also traumatic patellar dislocations, usually to the side with tearing of the medial retinaculum and the medial femoropatellar ligament; osteochondral flakes can also be peeled off. Pure soft tissue injuries that have already been reduced can usually be treated conservatively, but larger flakes should be refixed in good time. If a larger part of the capsule-ligament apparatus in the area of the medial retinaculum and the medial patellofemoral ligament tears, sutures should be used. More complex cases such as recurrent dislocations, genu valgum and persistent muscle imbalances must be treated accordingly.
Cause
- direct trauma
Predisposing
Risk factors for a more complicated course and indications for surgery
- Pre-existing ligamentous laxity, especially of the medial retinaculum of the patella
- Known tendency to patellar dislocation
Diagnosis
- Clinical examination
- MRI to examine the ligaments
Symptoms
- Painful restriction of movement, painful movement, even without strain
Complications
- without surgical treatment: Tendency to recurrent dislocations, osteoarthritis of the retropatellar cartilage
Therapy
- Pure soft tissue injuries without persistent dislocation or tendency to recur can be treated conservatively, including with adapted muscle training.
